By definition, a portable VHF is a piece of equipment that integrates in the same box the device, its antenna, its screen and keyboard, the microphone and the speaker. All-in-one that fits in your hand.

A remote microphone
In the catalog we find a remote microphone. The advantage of this is that you can keep the VHF on your belt (it comes with a clip) and have the microphone/speaker within easy reach of your mouth and ears. This type of installation is often seen on yachts when the captain is giving orders from the bridge and the sailors are maneuvering on the foredeck or quarterdeck.
The microphone/speaker is waterproof. It just allows you to listen and talk by pressing the PTT pedal. All settings including volume remain on the VHF. Moreover it is advisable to lock the keyboard (a long press on the key with the key) to avoid making involuntary changes of settings.
A headset and audio microphone
To really be at the heart of the action and stay focused on the maneuver, there are headsets or earpieces - similar to hands-free phone kits - that can be connected to the VHF. This way, the operator is always listening to the messages. To answer, since the VHF is not in duplex, a PTT box must be used between the headset and the VHF.
This box - a kind of big button impossible to miss - clips on the garment. Just press it to speak. Even with gloves on, it's easy.
